Mercy is seeking a Nurse Navigator to lead care coordination for both inpatient and outpatient populations. You will assess patient care needs, guide the care team, and ensure timely, safe, high-quality care aligned with Mercy standards.

Responsibilities include pre-op education, daily rounds, discharge planning, LOS management, and collaboration to optimize outcomes. RN license in state, 2–3 years’ experience, and BLS are required; BSN preferred and ACLS preferred.

Qualifikationen

• RN license in state of practice
• 2-3 years’ experience required
• BLS certification required
