Job summary

Mercy is hiring a Nurse Navigator to lead patient care coordination for inpatient and outpatient populations. The role focuses on pre-op education, daily rounds, discharge planning, and guiding care plans in collaboration with bedside nurses.

The Nurse Navigator will ensure timely, safe, high-quality care while balancing regulatory guidelines and Mercy Service Standards. Preferred BC/BS experience and strong teamwork are valued in this fast-paced care environment.

Qualifications

• Graduate from approved school of nursing.
• Current RN license in state of practice.
• 2-3 years’ experience required.
• BLS (Basic Life Support) certification.
• ACLS (Advanced Cardiac Life Support) preferred.

Responsibilities

• Provide pre-op education to patients and families.
• Conduct daily rounds and assess patient needs.
• Coordinate discharge planning and post-acute care.
• Collaborate with care team to optimize LOS and utilization.
• Ensure care is timely, safe, and aligned with Mercy standards.
