Job summary

Mercy Health System is seeking a Nurse Navigator to lead patient care coordination for inpatients and outpatients. The role assesses care needs, makes recommendations based on illness severity, and ensures timely, safe, high-quality care while coordinating with bedside nurses.

The Nurse Navigator supports discharge planning, length of stay management, and utilization, driving a seamless care model and collaboration across teams.

Qualifications

• Graduate from approved school of nursing.
• Current RN license in state of practice.
• 2-3 years' nursing experience required.
• BLS certification required.
• ACLS preferred; BSN preferred.

Responsibilities

• Pre-op education.
• Daily rounding while the patient is in the hospital.
• Coordinate discharge disposition.
• Follow-up care with patients after discharge.
