Job summary

Mercy seeks a Nurse Navigator to lead and coordinate patient care across inpatient and outpatient populations. The role involves pre‑op education, daily rounding, discharge planning, and collaboration with bedside nurses to ensure timely, safe and high‑quality care.

The Nurse Navigator will promote a seamless care model, adhere to regulatory guidelines, and mentor care teams while supporting LOS management and utilization practices at Mercy.

Qualifications

• RN license in state of practice.
• Graduate from approved school of nursing.
• 2-3 years' experience required.
• BLS and ACLS required within 90 days of hire/transfer.
• Bachelor of Science in Nursing preferred.

Responsibilities

• Provide leadership and mentorship to direct patient care teams.
• Coordinate delivery of patient care across units.
• Pre-op education and daily rounding with patients.
• Coordinate discharge planning and LOS management.
• Ensure care aligns with Mercy standards and regulatory guidelines.

Education

RN license

Graduate from approved nursing program

BSN preferred

BLS/ACLS within 90 days
