Job summary

Mercy seeks a Nurse Navigator to lead the coordination of patient care across inpatient and outpatient settings. You will assess patient needs, guide care plans, and ensure timely, safe, high-quality nursing support in collaboration with bedside nurses and care teams.

Qualifications

• Graduate from approved school of nursing.
• Current RN license in Missouri.
• 2-3 years of nursing experience.
• BLS and ACLS certifications within 90 days of hire.
• BSN preferred.

Responsibilities

• Provide pre-op education.
• Daily rounding and direct patient care.
• Coordinate care plans with bedside nurses.
• Discharge planning for post-acute services.
• Follow-up care and performance improvement.

Education
