Job summary

Mercyhealth is seeking an RN Patient Navigator to coordinate patient care, facilitate transitions, and optimize resource use. The role supports improved outcomes through strong case management and collaboration with physicians, nurses, and discharge planners.

You will educate patients and families about chronic conditions, coordinate post-hospital referrals, and participate in on-call rotations to ensure seamless care across settings.

Qualifications

• Graduate of an accredited nursing program.
• Current KS RN license or state equivalent and BLS/CPR.
• BSN completion required or planned enrollment within agreed timeframe.

Responsibilities

• Coordinate and facilitate patient care to ensure appropriate services and efficient use of resources.
• Assess patients early to identify discharge planning needs and community referrals.
• Create individualized, patient-focused plans for continuing care.
• Collaborate with healthcare team to ensure timely discharge and complete patient education.
