Job summary

Mercyhealth Wisconsin and Illinois is seeking an RN Patient Navigator to coordinate patient care and ensure efficient utilization of resources across the care continuum. You will partner with the hospital team to plan discharge and arrange post-acute services.

The role requires an accredited nursing degree and an active RN license in the practice state, with BSN preferred. You will educate patients and families about chronic conditions and work to minimize readmissions.

Qualifications

• Active RN license in the state of practice.
• BSN preferred or in progress for enrollment in BSN Completion.
• Case management certification preferred.
• BLS/CPR certification required within 90 days.
• Experience with discharge planning and care coordination.

Responsibilities

• Coordinate case management and discharge planning to prevent delays and readmissions.
• Assess patients early to identify coordination and discharge needs.
• Create individualized care plans and arrange post-acute referrals.
• Collaborate with care teams to ensure timely discharge and education.
• Educate patients and families about chronic diseases using current nursing knowledge.
• Collaborate with staff to meet core measures prior to discharge.
• Conduct Utilization Management review and identify financial risks.
