Mercy is seeking a Nurse Navigator to lead and mentor nursing staff, coordinate patient care based on illness severity and service intensity, and ensure timely, high-quality care across inpatient and outpatient settings.

Responsibilities include pre-op education, daily rounds, discharge planning, and follow-up care, aligning with Mercy standards and regulatory guidelines. BSN preferred; ACLS preferred; 2-3 years of RN experience required.

Qualifications

• Graduate from approved school of nursing.
• Current RN license in state of practice.
• 2-3 years’ experience required.
• BLS (Basic Life Support) certification.
• ACLS (Advanced Cardiac Life Support) preferred.

Responsibilities

• Provide pre-op education to patients.
• Conduct daily rounding while patients are hospitalized.
• Coordinate discharge planning and post-acute follow-up care.
