Job summary

Life Care Center of Cheyenne is seeking an RN MDS Coordinator to coordinate and assist with accurate, timely MDS Assessments, CCAs, and Care Plans in line with CMS RAI Manual Regulations.

The role requires an active RN license, nursing degree, and 2 years of nursing experience, preferably geriatric. CPR certification is required. Expect collaboration with an interdisciplinary team and adherence to Life Care standards.

Qualifications

• RN license in good standing throughout employment.
• CPR certification upon hire or during orientation and maintained during employment.
• Geriatric nursing experience preferred.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by the MDS Assessment to the DON.
• Educate direct care staff regarding CMS RAI Manual or SNF Regulation updates affecting documentation.
• Review Interdisciplinary Comprehensive Care Plan and Final Validation Reports for CMS QIES submission.
