Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to oversee and submit MDS Assessments, CCAs, and Care Plans per CMS RAI guidelines. You will liaison with the DON and educate staff on regulatory updates to documentation.

Ideal candidates hold an RN license, a nursing degree, and have 2 years of nursing experience, preferably in geriatric care, with CPR certification current.

Qualifications

• Associate’s or bachelor’s degree in nursing from an accredited college or university.
• Active RN license in good standing in the applicable state.
• Two years of nursing experience; geriatric nursing preferred.
• CPR certification at hire or during orientation; must remain current.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary, accurate MDS Assessments, CCAs, and Care Plans per CMS RAI regulations.
• Report any changes in a patient’s condition identified by the MDS Assessment to the DON.
• Provide education to direct care associates regarding CMS RAI Manual updates that impact documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review CMS Reports to identify assessments completed or submitted late and develop processes to prevent reoccurrence.
