Job summary

Life Care Centers of America is seeking a dedicated RN MDS Coordinator to ensure timely completion and submission of MDS Assessments, CAAs, and Care Plans in accordance with CMS RAI Manual Regulations and all applicable laws.

The role requires an active RN license, 2 years of nursing experience (geriatrics preferred), and CPR certification on hire or during orientation. Strong knowledge of post-acute care documentation is essential.

Qualifications

• RN license in good standing in the applicable state.
• Two (2) years’ nursing experience; geriatric nursing experience preferred.
• CPR certification upon hire or during orientation; must remain current during employment.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans as per CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by the MDS Assessment to the DON.
• Educate direct care staff about CMS RAI Manual updates or Skilled Nursing Facility regulations impacting documentation.
• Review Interdisciplinary Comprehensive Care Plans and CMS Final Validation Reports; ensure timely billing and acceptance in CMS QIES system.
