Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to travel across Plains region facilities, ensuring accurate and timely MDS Assessments, CCAs, and Care Plans in line with CMS regulations.

The role requires an active RN license, 2 years of nursing experience (geriatrics preferred), and CPR certification. You will implement policies, coordinate with teams, and ensure documentation integrity across post-acute care settings.

Qualifications

• Two years’ nursing experience; geriatric nursing preferred.
• RN license in good standing in applicable state; CPR certification on hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff regarding CMS RAI Manual updates and facility regulations.
• Review Interdisciplinary Comprehensive Care Plans and finalize validations before billing.
• Attend PPS and other meetings; maintain confidentiality and professional conduct.
