Job summary

Life Care Centers of America seeks an RN MDS Coordinator to support the completion and submission of MDS Assessments, CAAs, and Care Plans in line with CMS RAI regulations. This float/PRN role travels to facilities in Michigan and Ohio or serves as needed.

The ideal candidate has an RN license, 2 years of nursing experience (geriatric preferred), and CPR certification. Strong documentation skills and CMS knowledge are essential, with a commitment to high-quality patient care.

Qualifications

• Associate’s or bachelor’s degree in nursing from an accredited college or university.
• Active RN license in good standing.
• Two years nursing experience, geriatric preferred.
• CPR certification at hire or during orientation.

Responsibilities

• Coordinate and assist with interdisciplinary MDS Assessments, CAAs, and Care Plans.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI updates affecting documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and ensure CMS QIES submissions before billing.
• Identify late assessments and develop processes to prevent recurrence.
• Participate in PPS, Triple Check, and other required meetings.
• Perform staff nurse functions as needed and maintain patient safety.
