Job summary

Qualifications

• Requires nursing degree and current state licensure.

Responsibilities

• Oversee resident assessment process including MDS assessments and care plans.
• Lead weekly PDPM meetings for skilled residents and monitor tool utilization.
• Track QM/QASP performance and provide data-driven recommendations.
• Provide PDPM and MDS training to new coordinators.
• Coordinate interdisciplinary participation in MDS completion and care planning.
• Ensure timely data entry and electronic submission of MDS assessments.
