Qualifikationen

• Must be in good standing with the State Board of Nursing and maintain required continuing education/licensing.
• Three years of supervisory experience preferred.
• Long term care experience preferred.
• Knowledge and experience with PCC is preferred.

Aufgaben

• Oversee the facility to ensure assessments are completed per guidelines, focusing on MDS and resident plans of care.
• Provide PDPM and MDS training to new MDS Coordinators.
• Coordinate interdisciplinary participation in completing the MDS and ensure regulatory timeframes are met.
• Maintain accurate schedule of MDS assessments and ensure proper reference dates.
• Enter data accurately and submit MDS electronically, verifying submissions and maintaining records.
• Lead resident care conferences and assist disciplines in formulating care plans.
• Evaluate care plans for completeness and individuality and revise as needed.
• Communicate level of care changes to all departments and coordinate necessary forms.
