Job summary

Qualifications

• BSN from an accredited Nursing school; in good standing with the State Board.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred; PCC knowledge a plus.

Responsibilities

• Oversee the facility's resident assessment process including MDS and care plans.
• Lead and participate in weekly PDPM meetings for skilled patients.
• Track PDPM tools usage and ensure accurate data entry and submissions.
• Provide PDPM and MDS training to new coordinators.
• Coordinate interdisciplinary MDS completion within regulatory timeframes.
• Schedule and conduct resident care conferences and ensure documentation completeness.
• Maintain confidentiality of resident information.
