Qualifications

• Requires graduation from an accredited School of Nursing; BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain required licensure.
• Three years of supervisory experience in long-term care preferred.

Responsibilities

• Oversee resident assessments per guidelines, including MDS and care plans.
• Lead weekly PDPM meetings and track utilization of PDPM tools.
• Monitor QM and QASP performance; provide data analysis and recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure timely completion of MDS assessments and PPS Medicare.
• Coordinate interdisciplinary participation in MDS and care planning.
• Maintain confidentiality of resident information and regulatory compliance.
