Job summary

Santa Rosa Post Acute in Santa Rosa, CA, is seeking a Nursing leader to oversee the facility’s MDS and resident assessment process. You will coordinate the MDS cycle, PDPM initiatives, and interdisciplinary care plans to ensure timely, accurate documentation and optimal resident outcomes.

You will supervise nursing staff, align assessments with regulatory standards, and provide education to new coordinators.

Qualifications

• Graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.
• Must be in good standing with the State Board of Nursing.
• 3 years of supervisory experience preferred. Long Term Care Experience Preferred.

Responsibilities

• Oversee the facility and ensure assessments are completed per guidelines.
• Lead MDS and PDPM activities and coordinate interdisciplinary participation.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure timely completion and accuracy of all MDS assessments.
• Maintain confidentiality of resident care information.
