Job summary

Life Care Center Westminster is seeking an RN MDS Coordinator to coordinate and submit MDS Assessments, CCAs, and Care Plans in line with CMS RAI Manual regulations. The role requires RN licensure in good standing and prior MDS experience.

Must have strong knowledge of LTC/SNF regulations and be able to educate staff on CMS updates impacting documentation. The position emphasizes teamwork and accuracy in a fast-paced post-acute setting.

Qualifications

• MDS assessments experience and knowledge of CMS RAI helpful.
• Active RN license in good standing.
• Two years of nursing experience; geriatric experience preferred.
• CPR certification by hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Communicate changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI updates affecting documentation.
• Review interdisciplinary care plans and CMS reports to ensure timely submissions.
