Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to coordinate interdisciplinary MDS Assessments, CCAs, and Care Plans in a skilled nursing setting. The role ensures accurate documentation in accordance with CMS RAI manuals and applicable laws.

The ideal candidate holds an RN license in good standing, a nursing degree (associate or baccalaureate), CPR certification, and at least two years of nursing experience, preferably in geriatric care.

Qualifications

• Associate’s or bachelor’s degree in nursing.
• RN license in good standing in applicable state.
• Minimum 2 years of nursing experience; geriatric experience preferred.
• CPR certification at hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary, accurate MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by MDS to the DON.
• Educate direct care staff on updates to CMS RAI Manual or SNF regulations affecting documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and attest that assessments are completed and entered into CMS QIES prior to billing.
