Job summary

Fundamental seeks an RN Assessment Coordinator (RNAC, MDS) in Texas. The role coordinates patient care from admission through discharge under the DON. The position emphasizes accurate MDS reporting, RAI workflow, and collaboration with the nursing team.

The candidate should hold an active RN license with 2+ years of nursing and long-term care experience, plus ICD-10 proficiency. A reliable vehicle is preferred for site responsibilities.

Qualifications

• Current, unencumbered RN license in state of practice.
• Minimum of 2 years direct hands-on nursing experience.
• Minimum of 2 years of long-term care experience.
• Preferably at least 1 year of RAI experience (MDS).
• Proficiency in ICD-10 coding.
• Reliable transportation.

Responsibilities

• Identifying/monitoring resident skilled services.
• Coordinating the Resident Assessment Instrument (RAI) process.
• Completing an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care.
• Monitoring patient/resident status to identify changes and progress toward care goals.
• Participating in the daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM).
• Establishing and maintaining relationships with the healthcare team and nursing management.
