Fundamental in Mitchellville, MD is seeking a Registered Nurse Assessment Coordinator (RNAC) under the Director of Nursing to coordinate patient care from admission through discharge.

Qualifications include an active RN license, 2+ years of direct nursing and long-term care experience, with preferred 1 year of RAI experience and ICD-10 coding proficiency. Excellent benefits and competitive pay offered.

• Must hold an active, unencumbered RN license in state of practice.
• Minimum 2 years direct hands-on nursing experience.
• Minimum 2 years of long-term care experience.

Aufgaben

• Coordinate resident care from admission to discharge.
• Identify/monitor resident skilled services.
• Coordinate the RAI process.
• Complete an accurate Minimum Data Set (MDS) and develop the interdisciplinary Plan of Care.
• Monitor patient/resident status for changes and discharge barriers.
• Participate in daily PPS Interdisciplinary Team Meeting.
• Maintain positive relationships within the healthcare team.
