Qualifications

• Current, unencumbered RN license required.
• Minimum of 2 years of direct hands-on nursing experience.
• At least 2 years of long-term care experience required.
• Preferred 1 year of RAI experience focused in MDS.
• Reliable transportation needed.

Responsibilities

• Monitor resident skilled services.
• Coordinate the Resident Assessment Instrument process.
• Complete Minimum Data Set and develop Plan of Care.
• Monitor patient/status for changes and barriers.
• Participate in daily interdisciplinary team meetings.
• Establish positive relationships within the healthcare team.
