Fundamental in Houston, TX is hiring a Registered Nurse Assessment Coordinator (RNAC/MDS). You will oversee patient/resident care from admission to discharge, coordinate the RAI process, and ensure accurate MDS documentation within the nursing team.

Qualifications

• Must have a current, unencumbered RN license in state of practice.
• Minimum of 2 years direct nursing experience.
• Minimum of 2 years long-term care experience.
• Prefer 1 year of RAI experience (MDS focus).
• Proficiency in ICD-10 coding.

Responsibilities

• Identify and monitor resident skilled services.
• Coordinate the Resident Assessment Instrument (RAI) process.
• Complete an accurate MDS and develop the interdisciplinary Plan of Care.
• Monitor patient status for changes and barriers to discharge.
• Participate in the daily PPS Interdisciplinary Team Meeting.
• Maintain positive relationships within the healthcare team.
