Life Care Centers of America is seeking an RN MDS Coordinator to coordinate and assist with accurate MDS Assessments, CCAs, and Care Plans per CMS RAI guidelines in a long-term care setting.

The role requires an active RN license, a nursing degree, and at least two years of nursing experience, with geriatric experience preferred. CPR certification is required or obtained during orientation.

Qualifications

• Associate’s or Bachelor’s degree in nursing from an accredited college or university.
• Current RN license in good standing in the applicable state.
• Two years of nursing experience; geriatric nursing experience preferred.
• CPR certification at hire or during orientation and must remain current.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary, accurate MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by the MDS Assessment to the DON.
• Provide education to direct care associates regarding CMS RAI Manual updates or Skilled Nursing Facility Regulations that impact documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and attest that all assessments are completed and accepted into the CMS QIES system prior to billing.
