Job summary

Life Care Centers of America in Melbourne, FL seeks an RN MDS Coordinator to oversee timely MDS assessments, CAAs, and care plans in line with CMS regulations.

The role requires an active RN license, 2 years of nursing experience (geriatric experience preferred), and CPR certification. You will collaborate with an interdisciplinary team to ensure compliant documentation and ongoing quality care.

Qualifications

• Must hold an active RN license in good standing.
• Associate's or Bachelor's degree in nursing.
• Minimum 2 years of nursing experience; geriatric experience preferred.
• CPR certification at hire or during orientation.

Responsibilities

• Coordinate and submit MDS Assessments, CCAs, and Care Plans per CMS regulations.
• Report changes in patient condition to the DON.
• Educate direct care staff on CMS updates affecting documentation.
• Review Interdisciplinary Comprehensive Care Plans.
• Review CMS Reports and prevent late assessments.
• Attend PPS/Triple Check meetings and assist in team operations.
• Perform staff nurse functions as needed.
