Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to lead interdisciplinary MDS assessments, CAAs and care plans in a compliant post-acute setting.

This role requires an RN with a nursing degree and an active license, plus 2 years of nursing experience; geriatric experience is preferred. You will coordinate CMS RAI documentation, educate staff, and ensure timely reporting with Life Care standards.

Qualifications

• RN with an active license and nursing degree; CPR may be required.
• At least 2 years of nursing experience; geriatric experience preferred.
• Knowledge of CMS RAI, MDS/CAAs, and post-acute care regulations.

Responsibilities

• Coordinate and submit MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient’s condition to the DON.
• Educate staff on CMS RAI changes impacting documentation.
• Review interdisciplinary care plans and CMS reports.
• Participate in PPS meetings and other required meetings.
• Perform staff nurse functions as needed.
• Maintain patient confidentiality and professionalism.
• Demonstrate dependable, regular attendance.
