Job summary

University Park Care Center is seeking an RN MDS Coordinator to manage and submit MDS Assessments, CCAs, and Care Plans per CMS RAI rules and Life Care standards. You will coordinate with an interdisciplinary team and ensure timely, accurate documentation for regulatory compliance.

The role requires an RN license, an associate or bachelor’s degree in nursing, and at least two years of nursing experience, with geriatric experience preferred.

Qualifications

• Nursing degree from an accredited college or university.
• Active RN license in good standing.
• Two years’ nursing experience, geriatric nursing preferred.
• CPR certification on hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Review and attest CMS QIES submissions prior to billing.
• Educate direct care staff on CMS RAI Manual updates and regulatory changes.
• Participate in PPS, audit processes, and interdisciplinary team meetings.
• Maintain confidentiality and adhere to Life Care standards and conduct.
