Job summary

Life Care Centers of America in Massachusetts is seeking an RN MDS Coordinator to oversee interdisciplinary MDS assessments, CCAs, and care plans in line with CMS RAI regulations.

The position requires an active RN license, at least two years of nursing experience (geriatrics preferred), and CPR certification. You will work with a team to ensure accurate documentation, regulatory compliance, and patient safety.

Qualifications

• RN license in good standing.
• Associates or Bachelors in Nursing.
• Two years’ nursing experience; geriatric preferred.
• CPR certification by hire and maintained.

Responsibilities

• Coordinate and submit MDS Assessments, CCAs, and Care Plans per CMS RAI.
• Inform DON of changes in patient condition identified by MDS.
• Educate staff on CMS RAI updates affecting documentation.
• Review and attest CMS reports and ensure readiness before billing.
• Participate in PPS meetings and other required meetings.
• Provide patient care duties as needed.
• Maintain confidentiality and comply with policies.
