Job summary

Life Care Centers of America seeks an RN MDS Coordinator to oversee and support accurate MDS assessments, CCAs, and care plans in a post-acute setting. You will ensure timely submission according to CMS RAI rules and Life Care standards, while coordinating with the interdisciplinary team.

The role requires an active RN license, 2 years of nursing experience (geriatrics preferred), and CPR certification. You may participate in ongoing education and compliance activities as part of the team.

Qualifications

• RN license in good standing.
• Two years of nursing experience; geriatric nursing preferred.
• CPR certification by hire and maintained during employment.

Responsibilities

• Coordinate and assist with CMS MDS Assessments and Care Plans.
• Report changes in a patient’s condition identified by the MDS to the DON.
• Educate direct care staff regarding CMS RAI updates and facility regulations.
• Review Interdisciplinary Care Plans and Final Validation Reports for CMS CMS QIES readiness.
• Attend PPS and other required meetings and support billing readiness.
• Provide staff with competent nursing functions as needed.
• Maintain confidentiality and comply with company policies.
