Job summary

Elevance Health in New Jersey is seeking an LTSS Service Coordinator - RN Clinician to manage members' care plans across health and long-term services. This field-based role requires coordination with the care team and oversight of non‑RN staff to deliver timely services and align with state requirements.

The position is based in Hudson County, NJ, with a hybrid work arrangement and travel to client sites as needed.

Qualifications

• Must have RN license and be currently licensed in applicable state(s).
• HS diploma or equivalent required.
• Minimum 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a service coordination or case management role.

Responsibilities

• Identify members for high-risk care needs and coordinate with health care team.
• Develop, monitor, and revise individual care plans to optimize member health outcomes.
• Perform telephonic or in-person assessments to identify needs across physical health, behavioral health, social services and long-term supports.
• Coordinate with non-RN clinicians and supervise LPN/LVN, LSW, LMSW and others in care coordination.
• Interface with Medical Directors and Interdisciplinary Teams on care management plans.
