Job summary

Elevance Health is seeking a LTSS Service Coordinator - RN Clinician to work primarily in the field across Passaic County, NJ, with occasional meetings at a local office. The role focuses on comprehensive care coordination for members, including health and social services, under licensure guidelines.

Candidates must hold a valid RN license, have at least 3 years of experience with chronic conditions, and be comfortable traveling to client sites.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in applicable state(s) required.

Responsibilities

• Identify members for high risk complications and coordinate care with the health care team.
• Manage members with chronic illnesses to ensure cost effective and efficient utilization of health benefits.
• Develop and revise individual care plans in collaboration with the member and care team.
• Coordinate services and facilitate authorizations/referrals as appropriate.
