Job summary

Elevance Health is seeking a field-based LTSS Service Coordinator - RN Clinician in New Jersey to manage members' cases, develop care plans, and coordinate with non-RN clinicians across the care continuum.

The role involves telephonic or face-to-face assessments, collaboration with medical directors, and oversight of licensed staff; travel to client sites and occasional office attendance are required under a hybrid work policy.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience with chronic illnesses in a service coordination, case management or similar role.
• Current, active RN license in applicable state(s) required.
• State certification may be required by state law and/or contract.
• Associates/MA/MS in Health/Nursing preferred.
• Travels to worksite and other locations as necessary.

Responsibilities

• Identifies members for high risk complications and coordinates care with the health care team.
• Manages members with chronic illnesses to ensure cost effective utilization of health benefits.
• Obtains thorough member history to develop individual care plans.
• Establishes short and long term goals with the member and caregivers.
• Interfaces with Medical Directors and interdisciplinary teams on care management plans.
• Oversees non-RN clinicians to coordinate services for the member.
