Job summary

Elevance Health is seeking an LTSS Service Coordinator - RN Clinician in Middlesex County, NJ. This field-based role travels to client sites and supports care coordination across health and long-term services.

Responsibilities include telephonic or face-to-face assessments, developing care plans, and guiding non-RN clinicians within regulatory requirements. The position emphasizes collaboration with Medical Directors and the care team.

Qualifications

• HS diploma or equivalent required.
• Current, active RN license in applicable state.
• 3+ years in chronic illness/co-morbidity care coordination or similar role.

Responsibilities

• Perform telephonic or face-to-face clinical assessments to identify and manage member needs.
• Develop, monitor, and revise individual care plans across health and LTSS services.
• Coordinate with health care team to optimize member health care across the continuum.
• Provide direction to non-RN clinicians (LPN/LVN, LSW, LMSW) as appropriate.
• Interface with Medical Directors and interdisciplinary teams on care plans.
• Assist in authorizations/referrals within benefits structure and policies.
