The Elevance Health Companies, Inc. is seeking an LTSS Service Coordinator - RN Clinician to manage member cases across health and long-term services.

The role involves telephonic and in-person assessments, developing individualized care plans, and coordinating with a team of licensed professionals to ensure high-quality care for members with chronic conditions. The position supports a hybrid work model with field-based requirements; candidates should reside within commuting distance of Harrison

Qualificações

• HS diploma or equivalent.
• Minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role; or any combination of education and experience providing an equivalent background.
• Current, active valid and unrestricted RN license in applicable state(s) required.

Responsabilidades

• Manage the member's case within the scope of licensure and develop, monitor, evaluate, and revise care plans to optimize health care across the continuum.
• Perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination and management of member needs across physical health, behavioral health, social services and long term services and supports.
• Provide direction and oversight to non-RN clinicians participating in the member's case and coordinate services with medical directors and interdisciplinary teams.
