Job summary

Elevance Health is seeking an LTSS Service Coordinator - RN Clinician in Middlesex County, NJ. This field-based role travels to client sites and supports care planning, coordination, and assessments for members with chronic illnesses and disabilities.

The RN will oversee non-RN clinicians, develop care plans, and coordinate with physicians and care teams. A current RN license is required, with potential state certifications as needed.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience with individuals with chronic illnesses, co-morbidities, and/or disabilities in a service coordination/case management role.
• Current, active RN license in applicable state(s).
• May require state-specified certification based on state law/contract.

Responsibilities

• Manage member cases within licensure scope and develop/monitor care plans to optimize health care across the continuum.
• Perform telephonic or face-to-face clinical assessments for identification, evaluation and management of member needs.
• Coordinate with health care team and non-RN clinicians to implement care plans.
