Job summary

Elevance Health is seeking a Field-based LTSS Service Coordinator - RN Clinician in Hudson County, NJ. This role involves traveling to client sites to assess, coordinate, and manage members’ care plans in collaboration with the health care team.

The candidate must hold an active RN license in the required state, have at least 3 years of experience with chronic illnesses, and be capable of guiding non-RN clinicians within regulatory and contractual frameworks.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in applicable state(s) required.
• May require state-specified certification based on state law and/or contract.
• Associates/MA/MS in Health/Nursing preferred.

Responsibilities

• Identify members for high risk complications and coordinate care with the health care team.
• Manage members with chronic illnesses to ensure cost-effective and efficient utilization of health benefits.
• Obtain thorough member history to develop an individual care plan and establish goals with the member and care team.
• Interface with Medical Directors and Inter-Disciplinary Teams on care management plans.
• Provide direction to non-RN clinicians in coordinating services and input on performance.
