Elevance Health seeks an LTSS Service Coordinator - RN Clinician in New Jersey. This field-based role travels to client sites with occasional office attendance for meetings and training.

The RN guides care plans, coordinates with non-RN clinicians, and ensures licensure compliance while optimizing member health across the continuum. The position emphasizes telephonic or face-to-face assessments, collaboration with medical leadership, and coordination of long-term services and supports.

Qualifications

• 3+ years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in applicable state(s) required.
• May require state-specified certification based on state law and/or contract.

Responsibilities

• Identifies high-risk members and coordinates care with the health care team.
• Manages members with chronic illnesses to ensure cost-effective utilization of health benefits.
• Obtains thorough member history to develop an individual care plan.
• Establishes short and long-term goals with the member and care team.
• Develops care plans and oversees service authorizations/referrals as appropriate.
• Interfaces with Medical Directors and Interdisciplinary Teams on care plans.
• Oversees non-RN clinicians in coordinating services and evaluates their performance.
