Qualifications

• Associates of Science (A.S) degree in nursing is required.
• RN license to practice independently in MA is required.
• Experience in care coordination or dual-eligible populations is preferred.

Responsibilities

• Engage enrollees in homes and community settings to build a complex care management relationship.
• Coordinate with healthcare providers and community resources to ensure seamless care transitions.
• Perform comprehensive assessments and develop care plans with ICT collaboration.
• Monitor enrollee utilization of long-term services and supports to maintain preferred locations of care.
• Educate members on benefits, preventive care, and managing chronic conditions.
