Job summary

CareSource is seeking an Integrated Care Clinical Manager for Massachusetts to supervise home-based care and coordinate services for dually-eligible enrollees with complex needs. This role emphasizes integration of health services and community resources, with travel to engage enrollees in-home visits and care planning.

The position requires an RN license and an AS in Nursing, with experience in care coordination or case management, and a willingness to work across multiple settings to improve

Qualifications

• Associates degree in nursing from an accredited program is required.
• RN license to independently serve people with complex needs.
• Care coordination or case management experience preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical field/community-based training is a plus.

Responsibilities

• Engage with enrollees in homes and community settings to manage complex care.
• Coordinate with healthcare providers and community resources to ensure seamless care transitions.
• Perform comprehensive assessments and develop care plans with the ICT.
• Lead the interdisciplinary care team to address medical and social needs.
• Travel regularly to member, provider, and community visits.
• Advise on utilization of long-term services and supports and advocate for enrollees.
