Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager to oversee complex care management for dually-eligible enrollees in their homes and community settings. You will lead an interdisciplinary team, coordinate services, and ensure high-quality, person-centered care that improves health outcomes and quality of life.

The role requires an RN with dual-eligible care experience, strong communication, and travel to community sites in the Commonwealth as needed.

Qualifications

• RN licensed in MA
• Experience in care coordination or case management a must
• Experience with dual-eligible populations preferred
• Medicaid/Medicare knowledge preferred

Responsibilities

• Engage enrollees in home and community settings to build complex care plans
• Lead the interdisciplinary care team to develop holistic plans
• Coordinate with providers and community resources to ensure seamless care transitions
• Monitor enrollee utilization of long-term services to support independent living
