Job summary

Affinity Health Management is seeking a licensed Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities in New Jersey. The ideal candidate has strong community health education experience and excellent nursing skills.

The RN will assess patients and families, develop and revise care plans, administer medications, educate families, and work with an interdisciplinary team to ensure high-quality, compassionate care in home settings.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in good standing in New Jersey.
• Experience in public health, home care or hospice preferred.
• Excellent observation, verbal and written communication skills.
• Ability to lift and transfer patients; physical stamina.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide skilled nursing care using the nursing process.
• Develop, implement, and revise plans of care with patient and family.
• Administer medications and treatments as prescribed.
• Educate and counsel patient and family; ensure safe home environment.
• Document clinical notes and communicate with physicians and care team.
• Participate in on-call duties and coordinate with interdisciplinary group.
