Job summary

Affinity Hospice Management LLC is seeking a dedicated Registered Nurse to plan, coordinate, and direct compassionate hospice care in home settings across New Jersey.

You will perform comprehensive patient assessments, develop care plans, administer medications, educate families, and collaborate with an interdisciplinary team. A valid NJ RN license and strong communication skills are required, with experience in hospice, home care, or public health preferred.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice in New Jersey.
• Minimum 1 year of nursing experience within the last 3 years; or BSN with 2 years’ experience (preferred in public health/home care/hospice).
• Supervisory experience not required; may supervise hospice aides.
• Post-offer background checks required by regulations.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Provide nursing care using the nursing process.
• Develop and revise care plans with patient and family.
• Administer medications and treatments as prescribed by the physician.
• Educate patient and family; provide self-care and safety guidance.
• Coordinate with physician and interdisciplinary team; on-call as required.
