Job summary

Affinity Hospice Management LLC in New Jersey seeks a registered nurse to plan, organize and direct hospice care in patients' homes and communities. The RN conducts comprehensive assessments, develops and revises care plans, administers medications per physician orders, and counsels patients and families on self-care and comfort.

Ideal candidates hold an NJ RN license, have at least one year of nursing experience (preferably in home care or hospice), and demonstrate excellent communication

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice in New Jersey.
• Minimum one year of professional nursing experience within the last three years; or a BSN with two years' experience (preferably in public health, home health, or hospice).

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments.
• Develop, implement and revise care plans with patient and family.
• Administer medications and treatments per physician orders.
• Provide patient and family education on self-care and comfort measures.
• Document and communicate patient status with physicians and interdisciplinary team.
• Participate in on-call duties and ensure equipment and services are arranged.
• Maintain safety and quality through continuing education.
