Job summary

Affinity Hospice Management LLC in New Jersey is seeking a registered nurse to plan, organize and direct hospice care in home and community settings. The RN will rely on nursing assessment and care planning to meet patient and family needs.

The role requires leadership in the nursing process, coordination with physicians and the interdisciplinary team, on-call rotation, and strong communication and caregiving skills to support comfort and quality of life.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in good standing in New Jersey.
• 1 year of recent professional nursing experience, or BSN with 2 years' related experience.
• Experience in public health, home care, or hospice preferred.
• Valid driver’s license with insured vehicle.

Responsibilities

• Perform initial and ongoing comprehensive patient/family assessments for hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient, family, and team.
• Administer medications and treatments per physician orders.
• Educate patients and families on care and self-care.
• Maintain accurate clinical notes and communicate changes to the physician.
• Coordinate with community health teams and report safeguarding concerns.
• Participate in on-call duties and ensure equipment and services available.
• Supervise hospice aides when required and pursue ongoing professional development.
• Participate in quality improvement activities.
