Qualifications

• Graduate of an accredited school of nursing
• Registered Nurse with a license in good standing to practice in the state
• One year of professional nursing experience in the last three years or a baccalaureate degree with two years’ experience (preferred)
• Management experience not required; may supervise hospice aides
• Offer contingent upon satisfactory references and criminal background checks
• Excellent observation, verbal and written communication skills, problem solving, basic math skills
• Ability to lift, position, and transfer patients; valid driver's license and insured vehicle

Responsibilities

• Completes initial comprehensive and ongoing assessments of patient and family to determine hospice needs
• Provides professional nursing care using the nursing process
• Writes and initiates plan of care; re-evaluates patient and family needs
• Initiates and revises plan of care as patient status changes
• Develops a care plan based on nursing diagnosis and family involvement
• Administers medications and treatments as prescribed
• Counsels patient and family in meeting nursing needs and self-care
• Provides health instructions and maintains safe patient environment
• Supports continuity of care with family/caregiver and interdisciplinary team
• Participates in on-call duties and quality improvement activities
