Job summary

Elevance Health is seeking an RN Telehealth LTSS Service Coordinator to manage members' cases within licensure and develop care plans that optimize health across the care continuum. This role may supervise non-RN clinicians and coordinate across care teams.

Ideal candidates hold an RN license with 3+ years in chronic illness care, and are eligible to work in a hybrid/remote capacity from the tri-state area (NY, NJ, CT). In-person training sessions may be required.

Qualifications

• Requires an RN and minimum of 3 years of experience with chronic illnesses in a Service Coordinator, Case Management, or similar role.
• Current, unrestricted RN license in NY required.

Responsibilities

• Perform telephonic clinical assessments to identify and manage member needs.
• Identify high-risk members and coordinate care with health care team.
• Manage chronic illnesses to optimize health benefits.
• Develop and monitor individual care plans.
• Coordinate authorizations/referrals within benefits structure.
