Job summary

Elevance Health in Indianapolis, IN is seeking an LTSS Service Coordinator - RN Clinician to manage a member's care in the field. The role requires traveling to client sites within Allen County and surrounding areas, with occasional office attendance as needed.

You will develop, monitor, and revise care plans, perform telephonic or in-person assessments, and coordinate with non-RN clinicians, physicians, and care teams to optimize health outcomes for members with chronic conditions.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience working with individuals with chronic illnesses or disabilities in a service coordination or case management role.
• Current, active RN license in applicable state(s) required.
• State-specific certification may be required by state law or contract.

Responsibilities

• Develop, monitor, evaluate, and revise the member's care plan to meet needs across the care continuum.
• Perform telephonic or face-to-face clinical assessments for identification, evaluation and management of member needs.
• Coordinate with non-RN clinicians (LPN/LVN, LSW, LMSW, etc.) and with Medical Directors or Physician Advisors on care plans.
• Identify high-risk members and collaborate with the health care team to optimize utilization of benefits.
• Provide direction to non-RN clinicians and monitor their performance related to member care.
