Job summary

Elevance Health is seeking a LTSS Service Coordinator – RN Clinician to manage member care plans in the field, traveling to client sites with occasional office attendance. The role emphasizes person-centered care across physical health, behavioral health, long-term services and supports.

You will perform assessments, develop goals with members and their support network, and collaborate with medical directors and care teams to optimize outcomes.

Qualifications

• Requires high school diploma or GED and at least 3 years in a role like Service Coordinator or Case Management.
• Current, active, unrestricted RN license in Ohio.
• BA/BS in Health or Nursing preferred.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify, evaluate, coordinate and manage member needs.
• Manage members with chronic illnesses to ensure cost effective, efficient utilization of health benefits.
• Obtain thorough member history to develop individual care plans.
• Set short and long term goals with members and care team.
• Develop care plans and ensure access to services within benefits structure.
• Assist with authorizations/referrals for services as appropriate, within policy.

Education
