Job summary

Elevance Health seeks an LTSS Service Coordinator RN Clinician (RN Case Manager) to manage a member's care plan across the health and long-term services continuum in Wake and Cumberland counties, with travel to client sites and occasional office attendance.

The RN conducts face-to-face assessments, identifies high-risk members, and collaborates with families, physicians, and care teams to set goals and coordinate services within benefits.

Qualifications

• Requires an RN and minimum of 3 years of experience with individuals with chronic illnesses, co-morbidities, and/or disabilities in a service/coordinator role.
• Current, unrestricted RN license in applicable state(s) required.
• State-specified certification may be required based on state law and/or contract.

Responsibilities

• Identify high-risk members and coordinate care with the health team.
• Develop and monitor the member care plan across health and long-term services.
• Facilitate authorizations/referrals for services within benefits structure or via approved arrangements.
• Supervise non-RN licensed professionals coordinating services for the member.
